CFBISD Offers Immunization Clinics
Passport Health/Staying Healthy Medical Services will be conducting student vaccination clinics on the following dates:
July 25th from 5-7 p.m.

Aetna, Blue Cross Blue Shield, Cigna, Humana & United HealthCare PPO plans will be accepted. Medicaid & CHIP are also accepted. You must bring a photo copy of your insurance card and primary insured’s driver’s license. All charges will be filed to your insurance carrier. For those with no insurance, $10.00 per vaccine to a maximum charge of $40.00/student. A parent or guardian must accompany students under 18 years old.
A CURRENT SHOT RECORD (in the English language) IS REQUIRED FOR ALL CHILDREN
Vaccines available: Varicella, DtaP, Hepatitis A, Hepatitis B, HIB, HPV, Meningitis, Meningitis B, Mumps, Measles, Rubella, Polio, HPV and TDaP
